Mechanical failure, pilot error blamed in fatal Duke Life Flight crash

The pilot of a Duke Life Flight medical helicopter that crashed in northeast North Carolina more than three years ago, killing all four people aboard, likely got confused by an array of emergency indicators about a problem with one of his two engines and mistakenly shut down the other one, according to federal investigators.
