Duke University Health System hired a chief patient safety officer. The job goes to Karen Frush.

Frush is chief medical director for children's services at Duke Hospital. The position was created after an organ transplant led to the death of 17-year-old Jesica Santillan.

In February 2003, a surgeon accidentally transplanted a heart and lungs of the wrong blood type into Santillan. Doctors transplanted a second set of organs into her, but she died soon after.

Frush's main responsibility will be to ensure that the health system adheres to safety standards.
