A facial plastic surgeon convicted of involuntary manslaughter in a drunken driving case can apply to get his medical license back.

Dr. Raymond Cook surrendered his license shortly after the Sept. 11, 2009, crash that killed Elena Shapiro, 20. Cook was sentenced to at least three years in prison.

On Wednesday, the state Medical Board said Cook would be eligible to reapply for his license on Sept. 11 of this year.
