The North Carolina Department of Labor on Tuesday released its report into the death of a ramp worker this summer at Raleigh-Durham International Airport.

Lufuluabo Kalonji was driving a small airport vehicle when it hit a parked American Eagle Airline plane on July 31. Kalonji was crushed between the two.

American Eagle Airlines has been fined $6,300 for occupational violations for what it classifies as serious hazards that could increaste the potential for injury resulting from an accident.

Among the violations: name plates or markings for powered industrial trucks were not in place; the employer did not ensure that each operator had successfully completed training for operating the trucks; and the training did not consist of a combination of formal instruction, practical training and evaluation.
