<b>RALEIGH, N.C.</b> — A former WakeMed Hospital nurse is charged with obtaining a controlled substance by fraud/forgery, according to police records.

WakeMed official said Atira Breanna Collins, 28, 2536 Blackwolf Run Lane in Raleigh, is no longer an employee there. It is unclear how long she worked at the hospital.

Collins was arrested on Nov. 9, according to an arrest warrant.
