Audit: Mismanagement costs NC Medicaid system millions

Inefficient management and lax oversight of contracts led to North Carolina spending much more to administer its Medicaid program than similar-size states, officials said Thursday.

State Auditor Beth Wood said North Carolina's administrative costs are 38 percent higher than the average of nine states because of "structural flaws" in how the Department of Health and Human Services operates the Medicaid program. The higher costs translate into an extra $180 million in North Carolina.

Although the Division of Medical Assistance is primarily responsible for processing Medicaid claims, two-thirds of the administrative costs were racked up by 10 other DHHS divisions, Wood said. Also, half of DMA's administrative costs were tied to contracts that weren't properly monitored.

Other problems found included a lack of budget forecasting – Medicaid spending was $1.4 billion over budget from 2009 to 2012 – and outright flouting of state law. Wood cited one instance where DMA withheld $131 million in drug rebate funds that were supposed to be transferred to the federal government and another where nursing homes received $13 million in inflationary adjustments.

DHHS Secretary Aldona Wos vowed changes in department operations under her watch.

"Cost overruns will not be tolerated and will not be accepted," she said. "There is a budget for a reason."

Wos said Carol Steckel, who was recently hired to run the Medicaid program, is already reviewing operations and establishing new policies and procedures.

Gov. Pat McCrory said his administration is working to fix what he called "a broken system," especially since DHHS is trying to implement new computer systems by this summer.

"Every dollar that is mismanaged at DHHS for Medicaid is $1 less that is available for medical services, education or things like road and bridge repair," McCrory said. "She cannot make good decisions and neither can I if we have bad information, and right now, there's a lot of bad information."

While McCrory said he hasn't made a decision about expanding Medicaid to accommodate people under the federal Affordable Care Act, Senate President Pro Tem Phil Berger said the mismanagement uncovered by the audit proves the state isn't ready to do that.

Legislation in the Senate would block any Medicaid expansion under the federal law and prohibit the state from establishing its own health insurance exchange, which would provide an online marketplace for people to shop for health coverage if they don't have an employer-sponsored plan.

House Speaker Thom Tillis said audit results are disappointing, if not surprising.

"The audit is further proof of inefficiencies and mismanagement at many levels of the Medicaid system that cannot be tolerated. It is apparent that improvements must be made to ensure that individuals dependent on Medicaid are adequately served by the system," Tillis said in a statement.

Lets dont forget about the dozens of people who get welfare for life.

I find most noted, was the fact of multiple structure "two-thirds of the administrative costs were racked up by 10 other DHHS divisions ".... Hopefully, this group will take her finding and do something about it, since last time it was just as bad, yet no one was fired and it appears they just kept right on abusing the system.

I further think if you qualify for medicaid help then you also need to pass a MANDATORY drug test too!

I would take it one step further. They should be tested for nicotine use and if found to be dependent on it, make quitting smoking a requirement to get welfare. Spend the money on your children, not on your habit!

Let's see all the folks out there who are getting Medicaid who don't need it and don't qualify, but yet they get it.

Case in point was a friend of mine whose ex wife got Medicaid for the children,...despite the fact that (1) she received nearly $1000 a month in child support; and (2) she remarried and soon after, her new hubby inherited nearly a quarter of a million dollars. But yet she received Medicaid for two children!

You mean cost the taxpayer, surely? And we have been told many times that the budget was cut to the bone.

Think of the money that could have been spent on education!

I knew a women who had medicaid for her and her two boys because she was a single parent and low income. This women got her teeth whiten for free. I had a job with insurance and couldn't afford to get my teeth whiten, what is fair about this?

And people on here have insinuated time and again that there is very little fraud/waste in Medicare/Medicaid- come on, wake up; I would say this proves that the fraud/waste for Medicaid in NC is significant. And you can safely assume that if there is 38% waste with regards to admin. costs in NC that there is also significant fraud/waste at the federal level as well; just think about 30% of your tax dollars paid towards DHHS at the federal level being wasted (That's $261 Billion per year). DHHS at the federal level has the same mindset and mentality that they do at the state level- it's inefficient and mismanaged. Still think we don't have a fiscal management and spending problem? This could also explain why NC's state income taxes are some of the highest in the US- higher than VA, PA, NY, etc.

The auditor puts together great reports but NO ONE IS EVER PROSECUTED NOR HELD RESPONSIBLE. So why do we even need an auditor?

well a few years ago while I was providing health insurance for my step children I was informed that were still being covered by medicaid. When I called I was told that is how the system is. Wouldn't it save money if people who have health insurance weren't on medicaid? I was told it was by the household not individual. CLearly this is an easy way to save some money. Benefits should also be earned and have a time peirod. People should not be allowed to live on medicaid but utilize it to help, that is what it is meant for to help not provide!

Bev did not have to be the one to request this audit. The Legislature could have done that.

No don't tell me the democratic lead government mismanageed funds for the poor and downtrodden. Say ITs Not SOOOOOOOO.

Gee what a shocker, and on Bev's watch to boot, how could this be?

Surely it was Bev who started this audit? Give her credit for that, but like she did with the nursing home problem, she kicked it into the next administration rather than deal with it. I wonder if she with teach those methods at Harvard in her stay there, and talk about political appointments to the Supreme Court as a lame duck and her hypocrisy in doing that. I'm a Democrat, still, but really we are democrits at times. I'm hoping McCrory does wonders, but our state has been run and let go. One party rule for over a century was not good for NC.

Medicaid pays for dental, vision, health for children, 100%. Children are well cared for in terms of public money spent on them.

It's not easy to qualify for medicaid as an adult. Your income can't be over 800 or so a month. Most people on disability make just over the cut off point.

They hire lazy, angry people who could not find jobs elsewhere. They cannot keep director-level positions filled. Management is marking time till retirement. You will find no one in the building after 5. And they bellyache about not getting raises. Truly you would not believe what else goes on there with HR's knowledge and management's blessing.

Every audit that has been done on state agencies in recent years points to fraud and waste - has ANYONE been fired?

I hope McCrory makes a few heads roll.

"That and fraud too! My brother in law makes over 100K and he and his family are on Medicaid. I have been reporting him to DHHS for the past 2 1/2 years and yet they have done nothing. His son was just in the ICU for 2 weeks and that bill was well into the hundereds of thousands that was picked up by the state. To make matters worse his "caseworker" is the one that helps him get around "the system." Family or not. I have no sympathy for those who abuse the system. I am just so ticked off that in 2 1/2 years DHHS has done NOTHING and I am giving them all the info on a silver platter."

Not trying to be funny but.... Does your nephew have a history of medical problems? Seems to me that maybe your inlaws may need the help especially with hospital/medical bills in the "hundreds of thousands" and his $100,000 salary. Keep in mind, they are family and Karma can be a you know what? Abusing the system is one thing and needing the HELP is another!

Here's why: they hire lazy people who then cannot get fired! And since their mentality is 'it's not MY money, why should I care?' it's even worse.

I worked w/ a guy once a while back who lived w/ the mother of his 3 kids but would NOT get married because the $$ would dry up! some people are just disgusting and should have to pay back every single penny they get for carp like this. And enough is enough's BIL.....KEEP reporting him, write letters, make phone calls don't stop

Wait, didn't obamacare eliminate all the fraud in medicaid?

Remember when we were told that their budget was cut to the bone? As always the law abiding tax paying citizen gets the shaft.
