I just took the assessment and it took less than 10 minutes and actually included some interesting heart health facts and statistics.
You do not have to set up an on-site assessment to get the coupon. I just clicked that I did not want the free follow-up screening and then it had me enter my address and Lowes Foods fresh rewards number. The final screen indicated I would receive my coupon in 2 to 3 weeks.
