Question: A Fortune 500 company that I had been employed by for over 16 years, has forged documents to obtain my medical information. The United States Department of Health & Human Services, Office for Civil Rights, is responsible for enforcement of this federal law. Its website is www.hhs.gov/ocr/hipaa.
